I have received, read and agree to abide by the rules and conditions relating to the booking and use of facilities (www.cryptonthegreen.net) and I confirm that I am hiring the Crypt either on my own behalf or as a member and representative of the organisation named above hiring the Crypt. I agree to pay a first deposit of 25% of the total cost as well as a separate £200 deposit against the failure to comply with the rules and conditions as stated. An invoice will be sent to you following receipt of this form (Please make cheques payable to St James Church Clerkenwell PCC)
